Chinese community church vacation bible school RELEASES
Registrants are required to agree to the following releases upon registering for Vacation Bible School.
LIABILITY RELEASE
In consideration for being accepted by Chinese Community Church for participation in its Vacation Bible School on July 21-25, 2025. I do hereby release, forever discharge and agree to hold harmless the Chinese Community Church and the directors thereof from any and all liability, claims or demands for personal injury, sickness or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the participant that occur while said person is participating in the above-described activity. The registrant further hereby agrees to hold harmless and indemnify said church, its directors, employees and agents for any liability sustained by said participant, including expenses incurred attendant thereto.
The registrant further consents to the administration of first-aid and/or doctor’s care, or any other form of medical treatment necessitated by illness or injury that may require the same. In the event of the necessity of such care or treatment as heretofore described, the undersigned agrees to hold harmless and indemnify said church, its directors, employees and agents from any acts of malfeasance, and/ or failure to act on the part of those chosen to administer medical care on behalf of the participant.
VIDEO AND PHOTOGRAPHY RELEASE
I hereby grant permission to the rights of my child’s image, likeness, and voice captured at the Chinese Community Church’s 2025 Vacation Bible School without payment or any other consideration. I understand this content may be edited, copied, exhibited, and published, and waive the right to inspect or approve the finished product. Additionally, I waive any right to royalties or other compensation arising or related to the use of this content. I also understand that this content may be used in diverse educational settings within an unrestricted geographic area in printed materials and on online. As the registrant, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any claims against any person or organization utilizing this material for educational purposes.
Non-acceptance of the Video and Photography Release must be stated in writing and emailed to cccvbs065@gmail.com. We will make our best effort to not publish any images or videos of your child. This includes not including your child in the VBS group photo.
